Depression after migration: when does homesickness go beyond cultural grief?

Migration often begins with hope, new choices and the possibility of a different future. Yet even a successful, voluntary move can involve loss, loneliness, uncertainty and psychological exhaustion. From outside, life may look good: you live in a new country, have begun work or study and perhaps achieved some goals. Inside, however, you may feel that you have lost some of your former energy, motivation or vitality.

Some days you miss your family and previous life. You may wonder, “Why am I not happy despite all these opportunities?” Homesickness, adjustment pressures and cultural grief are understandable after migration. But when sadness, lack of motivation and emptiness persist and affect sleep, relationships, work or everyday activities, homesickness may not be the whole explanation.

Is “depression after migration” a separate disorder?

It is not an independent psychiatric diagnosis. The expression usually describes depressive symptoms that emerge or become more intense during or after migration. Migration does not automatically cause depression. Migrants' experiences differ greatly, and many gradually adapt despite the pressures.

Nevertheless, migration can bring psychological, social and financial pressures at the same time. When these are intense, prolonged or unsupported, depressive symptoms may develop or return. Read also What is depression? Symptoms, causes and treatment.

Why can migration contribute to depression?

Migration is more than changing your address. In a short time, you may lose or have to rebuild your language environment, relationships, social role, career, lifestyle and sense of identity.

1. Distance from emotional support

At home, support may be woven into family visits, old friendships and familiarity with the language, neighbourhood and unwritten social rules. After moving, even simple tasks can require planning. Old friends are less accessible and new close relationships take time. This can intensify loneliness and feeling unsupported. See also the article on friendship and loneliness.

2. Loss of social or professional standing

Someone who had an established career, reputation and financial independence in Iran may need to start again from a less secure position. Unrecognised qualifications, barriers to employment, financial dependence or work below your skills can affect self-worth and professional identity. You may forget past achievements and interpret the present as personal inadequacy, although structural barriers and migration circumstances account for much of the difficulty.

3. Exhaustion from a second language

A new language involves more than vocabulary: processing quickly, understanding tone and jokes, and watching for mistakes all take effort. Continuous mental work can bring fatigue, reduced concentration and social withdrawal. You may feel you have lost your sociability or confidence when part of the experience is the strain of living in a second language. Read more about resilience and language barriers.

4. Prolonged uncertainty

Residence status, visas, employment, income, housing and your family's future can keep you on alert for months or years. Without enough recovery, initial anxiety may turn into exhaustion, hopelessness and loss of motivation. See migration anxiety and cultural grief.

5. Not belonging

Some migrants feel neither fully part of the new society nor able to return to their former lives as before. You may be seen as foreign abroad while relatives and old friends say that you have changed. Living between cultures can create uncertainty about identity and roots.

6. Discrimination or rejection

Racism, discrimination, mockery of an accent and not feeling accepted affect self-esteem and safety. Even apparently small incidents, repeated over time, can lead to withdrawal and discouragement.

7. Guilt towards family

Distance from parents, being unable to help, or having opportunities your loved ones lack may bring guilt. Positive experiences may trigger the thought, “I have no right to be happy while my family is struggling.” See migration-related guilt.

Why may depression appear later?

The first months can be energetic and busy: finding housing, arranging documents, learning the language and beginning work or study leave little room for feelings. Your mind may be focused on survival and practical problem-solving. As the initial demands ease, feelings that had no space to be processed can surface.

Symptoms may therefore emerge months or longer after migration. A delayed reaction does not mean ingratitude or that the move was wrong. Your mind may only now have room to register the changes and losses.

Homesickness, cultural grief or depression?

These experiences can overlap and their boundaries are not always clear. Homesickness often intensifies around particular people, places or memories. You may still enjoy some activities, plan for the future and feel temporarily better after a family call or something you enjoy.

Cultural grief concerns the loss of familiar surroundings, language, relationships, status and belonging. It can come in waves, especially around holidays and significant occasions. In depression, sadness, emptiness or loss of interest tends to be broader and more persistent. Activities that once felt enjoyable may no longer do so, and ordinary tasks can become difficult.

An important warning sign is that the difficulty continues on most days, across several areas of life, rather than only in one moment or situation.

Possible symptoms

Depression can involve visible sadness and crying, but also irritability, emotional numbness or physical complaints. Possible signs include sadness, emptiness or hopelessness most days; loss of interest or pleasure; low energy; disrupted sleep, early waking or oversleeping; appetite or weight changes; difficulty concentrating, deciding or remembering; worthlessness, failure or intense guilt; withdrawal; reduced motivation for language learning, work or study; agitation or sensitivity; persistent headaches, bodily pain or digestive symptoms; a sense that life or migration has lost its meaning; and recurrent thoughts of death or self-harm.

One or two symptoms do not establish depression. Duration, severity and impact matter. If several symptoms persist most days for around two weeks or longer, seek professional assessment.

Can a depression questionnaire help?

Questionnaires can help estimate symptom severity but do not replace a clinical interview or diagnosis. You can use the PHQ-9 for initial reflection, or the migration-related burnout self-assessment if prolonged migration pressures seem central.

Interpret results alongside physical health, psychological history, circumstances and daily functioning. A high score alone is not a diagnosis, and a low score does not always rule out difficulties.

What can help?

1. Do not minimise your experience

Choosing migration or having new opportunities does not invalidate distress. You can appreciate parts of your new life while grieving parts of the old one. Try to understand your feelings as a response to major changes rather than criticising yourself.

2. Build a small daily structure

Depression reduces motivation, and disorganisation can deepen symptoms. Start with a reasonably consistent waking time, one regular meal, gentle movement, a shower or a short outing. The aim is to restore a rhythm gradually, not force perfect functioning.

3. Reduce isolation gently

You do not have to wait until you feel completely better to connect. A short call, a walk with someone you know or a regular group activity can prevent deeper isolation. Small, predictable contacts are often more manageable than trying to create a large friendship circle immediately.

4. Keep connections with Persian language and culture

Adapting does not require erasing your previous identity. Music, literature, food, Persian and contact with people who share your background can provide continuity. The aim is not to remain only within a Persian-speaking community, but to build a tolerable bridge between your history and your new life.

5. Make goals small and realistic

Comparing your present with your previous professional or social position can intensify failure. Rebuilding is rarely linear. Instead of “I must become completely successful quickly,” try one job application, one class or one social contact this week.

6. Attend to physical needs

Irregular sleep, inactivity, poor nutrition and heavy alcohol or substance use can worsen symptoms. Physical care is not a complete treatment but is part of recovery. Physical illness and medication can also produce symptoms resembling depression; persistent fatigue, sleep changes or bodily symptoms merit medical review.

7. Seek professional help

If symptoms persist, disrupt functioning or have not improved with your own strategies, a psychologist, psychotherapist or doctor can help assess them. See when to consider psychotherapy.

How is depression after migration treated?

Treatment depends on severity, duration, history and preferences. Psychotherapy, medication or a combination may be considered. Therapy can address both depressive symptoms and the migration context: grief, family guilt, loneliness, residence or work uncertainty, lost professional status, conflicting cultural values, changing family and relationship roles, discrimination, and rebuilding identity and meaning.

A first language can make emotional and cultural details easier to express. Online therapy in Persian may suit Iranians abroad who cannot access a Persian-speaking therapist locally. Moderate or severe symptoms, previous depressive episodes or risk may also require psychiatric assessment. Medication decisions need individual professional evaluation.

Is returning to Iran the only way to feel better?

Depression may suggest a single immediate answer: “If I go back, everything will be fixed.” Returning may suit some people, but depression can narrow and darken how options appear. Where possible, make major decisions after considering your mental health, practical circumstances and the consequences of each choice. Therapy is not intended to persuade you to stay or return; it helps you decide with more understanding and less pressure from symptoms.

When is urgent help needed?

If you have thoughts of harming yourself or ending your life, a plan or means, feel unable to remain safe, or cannot meet essential daily needs, do not wait for an ordinary therapy appointment. Contact local emergency services and ask someone you trust to stay with you. Routine online therapy does not replace emergency care.

A final thought

Depression after migration does not mean weakness, ingratitude or failure. Sometimes, after months of adapting, there is finally room to face deep losses and changes. If sadness, emptiness, exhaustion or low motivation have affected your life for weeks, you do not have to manage alone. A professional conversation can help distinguish adjustment pressures from symptoms needing further treatment.

If you speak Persian and live outside Iran, you can ask about an online appointment on WhatsApp. Dr Mansoureh Bahrami has over 16 years of clinical experience in anxiety, depression, relationships, trauma and migration-related difficulties.
